Initial Visit: $60 - Includes 30 day supply of Phentermine(If, for some reason, patient is not a candidate for phentermine or if they desire an alternate medication, Rx will be provided and initial visit discounted to $35)

1) Comprehensive assessment = 30 minutes.

2) History (past medical history, current meds, habits, previous weight loss programs/diets tried, etc.

3) Objective measurements: weight, height, BMI, waist circumference, bust/hips/thighs/arms, body fat analysis.

4) Identify realistic weight loss goal

5) Diet prescription and nutrition counseling

6) Exercise prescription and counseling

7) Written educational/support material provided to patient.

8) Prescription for medication.  (Phentermine included in cost - $60 initial visit; $60 follow-up visits).

9)B12 injections Weekly injections included for weight loss patients. 

10) In some cases, lab orders to screen for diabetes, thyroid disorder, high cholesterol, other problems which may contribute to or complicate weight management (usually covered by insurance).  If patient has a primary care physician, this may be deferred to the PCP.

Follow up visits:

Every four weeks - $60 per visit - INCLUDES 30 DAY SUPPLY OF PHENTERMINE.(If, for some reason, patient is not a candidate for phentermine or if they desire an alternate medication, Rx will be provided and follow-up visit discounted to $35).  These visits are for assessment of progress, blood pressure monitoring, and coaching.  Body fat analysis will be repeated at discharge for program.

Usual duration of treatment with medication is 4-6 months, or less.
